Job Title: Nurse Practitioner (NP) or Physician Assistant (PA)

Summary: NP or PA to provide primary care services

Full Description:

We are looking for a NP/PA to provide primary care services for our patients such as well-woman exams, adolescent and sports physicals, cardiovascular and related visits for high cholesterol, high blood pressure, type 2 diabetes, pre-diabetes, weight management, smoking cessation; mental health visits for anxiety, depression, and other conditions.

The position is full-time, Monday to Friday, overtime exempt, and benefit eligible. Hours may change to include weekend and/or evening.

Required qualifications and application materials include:

• A degree from an accredited school of nursing and a Masters degree from an accredited Nurse Practitioner program. Current Minnesota RN license and nurse practitioner certification in Adult, Family, or Womens Health. Must be DEA registered and have billing privileges; or
• A Masters degree from an accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota Board of Medical Practice. Must be DEA registered and have billing privileges.
• Minimum of two years experience in primary care.
• Two letters of reference, including contact information of the reference.
